explorer.exe is not working

i use windows vista ultimate service pack one operating system.
nowadays,whenever i boot the machine and try to run a program or even try to watch a video,the second i open a folder,i get this message:(explorer.exe has stopped working)and then everything on the desktop dissapears & reappers again.
how can i fix this?
